How to complete a Care Request

If your loved one wants to continue living independently at home, but requires additional support, then the best way to find care that is tailored to their individual needs is to complete a Care Request.

A Care Request is a simple form that captures important information about your loved one's ability to perform daily tasks and identifies areas when they may need help. 

Follow this step by step guide to complete your first Care Request and start receiving personalised Care Matches for your loved one. 

Section 1: Tell us about your loved one

In order for us to provide personalised Care Matches for your loved one, then we need to know a few important details about them and their living circumstances.

1a. Known as *

Here we’d like you to tell us what your loved one is known as or their nickname, we’ll then use this as we build their care profile. We’re specifically not asking for their formal name. This is a mandatory field.

1b. Gender *

Please tell us your loved one’s gender so we can understand more about them and tailor their care matches. This is a mandatory field. 

1c. Age *

If we know your loved one’s age we can build a more precise picture of their care needs and the support they might require on a day-to-day basis. If you don’t know their exact age, approximate age is fine.

1d. Postcode *

Enter your loved one's postcode here - it’s essential for matching them with local home care providers and services.

Tick boxes - Tick all that apply

Here’s where we need a little more information about your loved one’s general health and living arrangements so we can understand their bespoke care needs.

Living alone - Tick this box if your loved one lives alone at home. 

Living with dementia - This will apply if your loved one has been diagnosed with a form of dementia, including Alzheimer’s. 

Wheelchair user - If your loved one uses a wheelchair, either occasionally or full time, check this box.

Deaf or hard of hearing - Tick this box if your loved one has been diagnosed as deaf or hard of hearing, meaning they struggle to hear. 

Partially sighted or blind - If your loved one has been diagnosed as partially sighted or blind let us know by ticking this box.  

Loneliness and/or isolation - Check this box if your loved one is affected by loneliness and/or isolation. 

Section 2: What type of help does your loved one need?

Now we’re getting into the more care-specific information. In order for us to provide the best care matches for your loved one we need to know about the type of support that is required throughout their daily routine. 

If you’re not sure exactly what your loved one can or can’t do, then always overestimate the support they need, you can always amend their needs later.

Tick all that apply. 

2a. Washing and personal hygiene

Your loved one may need help getting washed each day, perhaps they struggle to get in and out of the bath, or need help with using the toilet, or other personal hygiene tasks. If this applies then check this box.

2b. Getting dressed and undressed

Support getting dressed and undressed includes putting on and taking off clothes safely as well as changing them regularly. If your loved one would benefit from this, tick this box. 

2c. Meal planning and preparation

Meal planning and preparation is a huge part of the daily routine, but one that can easily go awry. Perhaps you’ve noticed your loved one not eating enough or finding it difficult to use the kitchen. They may also need support shopping for groceries. Tick if this sounds familiar. 

2d. Help with mobility and daily tasks

If your loved one has reduced mobility needs they may struggle moving around the house safely, for example using stairs and getting into and out of bed or a chair. A carer can support their daily mobility needs including completing exercises or attending appointments as well as other activities outside of the home. 

2e. Nursing care

Sometimes your loved one might have complex medical needs that require nursing care, if that’s the case then tick this box. We’ll ask for more specific details later. 

2f. Dementia care

Whether their needs are minimal or complex, a dementia carer can support your loved one with dementia providing engaging activities and helping them adapt. 

2g. Other

Everyone’s care needs are different, so if your loved one needs something specific or we haven’t included an element of care on our list then add them in below.  

Section 3: When do they need care to start?

In an ideal world, we would all have our later life car planned out, but that doesn’t always happen. 

Whether you’ve reached a crisis point and need care for your loved one immediately or are thinking ahead, let us know when you want the care to start. 

Please indicate whether you’re looking for care to start immediately, or on a future date which you can select using the calendar option. 

You can type the date or select it from the calendar by clicking the calendar icon. 

Please note that the format for dates is DD/MM/YYYY.

Section 4: What times of day?

Home care visits can happen throughout the day and night, to help your loved one maintain a daily routine by supporting them with certain activities.

Please select the times of day that your loved one needs help. 

Morning -  If your loved one needs help getting up and dressed then request care in the mornings. 

Afternoon - A carer visiting in the afternoon can help with daily activities, meal preparation and stick around for a cup of tea and a chat to check all is well. 

Evening - If your loved one needs support winding down for the night, an evening carer can help with bathing and getting to bed safely.

Overnight -  Sometimes care needs can come unexpectedly, so it’s important to know that help is close by. An overnight carer stays in the house all night.

Live-in care - A live-in carer will move into your loved one’s home to assist them at any hour of the day or night, so you can have complete peace of mind. 

Section 5: How often is care needed?

There is no one size fits all when it comes to care. 

All the support your loved one needs could fit into a few hours during the week, or perhaps you’re considering a more comprehensive care package. 

Remember that your loved one’s care needs are likely to change, so you can always update this section and get more care when it’s needed. 

Choose from weekdays (Monday to Friday) and/ or weekends (Saturday and Sunday) depending on when your loved one needs help. 

Section 6: How will your loved one be funding their care?

Care funding is one of the biggest worries for families when it comes to organising care. There are options for funding that come from the local authority and the NHS. 

Eligibility for funding depends on financial status and health needs. If you don’t meet funding criteria, you will pay for care yourself. 

Please tick the box that applies to your loved one’s funding situation. 

Self funding - People self funding their care are not eligible for financial support from the local authority. You are classed as a self funder if you meet the criteria of the upper capital limit (£23,250 or over in assets).  

Local authority with top up - Those using local authority funding with a top up are eligible for some funding from the local authority, but are paying a top up payment to access more expensive care. You may use local authority funding with top up if you fall between the upper and lower capital limits (less than £23,250 but more than £14,250). 

NHS/ICS/Personal Health Budget - A personal health budget is funded by the NHS or local authority to help you access the right care at home. 

Local authority - If your loved one is eligible for full local authority funding the adult social care team will put plans in place for their care from a list of agreed providers.

Completing the Care Request

If you need to go back to a previous section while completing the Care Request, simply click on the blue circle or the question next to it. 

You must complete every section of the Care Request to move through the form and get Care Matches.


Once you have completed your Care Request for your loved one, click the Find Care button to get their personalised Care Matches

If you’re not already logged in you’ll be directed to the login page so you can access your Care Matches from your Sweet Pea account.
